Sports Medicine
Trinity High School requires that you complete emergency information forms prior to each sports season.
By doing so, you provide important medical and emergency contact information to your athletic trainer, Brook-Lynne Marcotte at bmarcotte@trinity-hs.org or 603-668-2910 x 219, as well as a copy of an updated physical, which must be current within the past year. Additionally, please be aware of the fees associated with each sport.
Refunds due to injury may be requested within two weeks of the season from
the start of practice. After that time, there will be no refunds.
Lastly, all athletes must fill out the Permission to Participate, Code of Conduct, Concussion agreement, (links below) and Physical form before starting their sports season. PLEASE NOTE: There is not a link to a physical form There is no link/form for physicals because you must submit the physical form provided by your physician.

Two separate baseline concussion tests (ImPACT and C3 Logix) are required for all contact/collision sports prior to an athlete’s freshman and junior years. Transfers to THS and anyone who has not played a contact/collision sport previously are also required to complete the testing.
